Figures are attributable Medicare billing: each service is credited to a physician group only when the clinician who billed it belongs to exactly one group in CMS's Doctors and Clinicians register. A group is counted in every state its clinicians bill Medicare from, so state totals overlap and never sum to a single national figure the way the services and payments columns themselves do.
All figures are Medicare fee-for-service Part B only; Medicare Advantage claims are not included. County- and city-level totals are not published here — see methods for why.
